Fill out our quick assessment form online or our client assessment PDF, whichever is easier for you. A member of our team will contact you to schedule the completion of your assessment either over the phone or in person. Agency introduction by Life At Home Senior Care experienced team member that comes to your home to get to know you and discuss you care needs in more detail.
A person centered plan of care around your personal and health needs are created. We then schedule you to start services and we personally introduce you to your care provider.
Life At Home Senior Care Accepts the following forms of payment:
Medicaid waiver
Long term care insurance
Anthem Blue Cross Blue shield
Private Pay or out of pocket ( billed weekly or monthly)
Life at Home Senior Care does provide structured schedules for our clients.
There is a 4-hour daily minimum
We do provide 24/7 care for our clients if needed.
Here at Life At Home Senior Care, LLC our care providers go through a rigorous hiring process which include the following:
1. Care provides go through 2 interviews before being hired
2. Local and National Background checks
3. Drug screen and randomly screened throughout the year
4. Certifications are verified
5. Competency tested
6. Ongoing training throughout the year
